This is awesome! Thanks for sharing. It's not Tetris but recently I was able to acquire the prototype puzzle game 'Sushi Bar'.
The DS was the first console I bought with my own money, and Tetris DS was my gateway to Tetrisconcept and TGM; it's always nice to see this kind of what-it-could-have-been. Thanks a bunch !
