TGM probably stands for something else in Europe, just as TNT stands for both The New Tetris and an explosive.
TGM probably stands for something else in EuropeI don't think that "TGM" acronym stand more naturally to a puzzle game in Europe/EU or everywhere else except Japan...
in France TNT also stand for :
- dynamite (napoleon ?)
- a company delivery service
- Television Numerique Terrestre (more recently)

Uhh... no they're all TGM players. And there're more who aren't on there, mostly from mainland europe as well. For some reason France and Sweden are just full of them.

So 1. why was the decision made to make TGM a Japan-exclusive game, and 2. how are there so many players of a Japan-exclusive game on the other side of the globe from Japan?

Kotetsu nailed it. Though I'd add to #1:
TGM seems to exist in spite of TTC and not because of it. Also, Arika is a pretty small company. Put it all together and TGM is more or less stranded in Japan. http://www.tetrisconcept.net/forum/images/smilies/icon_sad.gif
Amusing sidenote: tetris_japan_finals is not the proper name of that video. It was originally distributed as death_800.mpeg from Arika's website. And it's quite possibly the weakest Tetris video to ever appear on Arika's site.

So why not rename it Flashpoint: The Grand Master (http://www.klov.com/game_detail.php?game_id=7823) and break away from the Tetris franchise?

You'll notice in the credits of Flashpoint that it traces back to ELORG also.
Now, yes they could name it something random, but I can't picture a serious game developer doing that. Also, I'm not sure but I think "look and feel" laws work differently in Japan.
